 Welcome to the TCN Behavioral Health web registration portal. With the addition of these pages, we are able to gather all of your registration information without the need for face-to-face interaction.

Completing these forms does not begin an appointment process. After completing the forms you must call TCN at 937-376-8700 to begin the appointment process.

Documents marked with a * will have special instructions below.

Please fill out and sign each document (Links to the left).

Please have copies of your ID and insurance card/s available. These can be attached to the document via a photo, or a file upload.

Mont Subsidy Application and Mont 100-2 are for Montgomery County Residents only.

The Document Upload form should only be used should we require more information.

The Christopher House/Recovery Housing Application should only be filled out by those requesting recovery housing.
